Appointments are typically 20-30 minutes in length.

What to Bring:

  • Valid Driver’s License

  • Glasses if needed for distance

  • Medication List (including dosages). Please note some medications require additional clearances from your PCP. If unsure if your medication requires a clearance form completed, please call/email us and we can advise you.

If you have sleep apnea or have had a sleep test in the past:

  • Sleep Apnea Compliance Report (if you have been diagnosed with sleep apnea). We are unable to download report from SD card in the office. Compliance report must include data since your last DOT physical was completed. Example: if your last physical was 1 year ago, the report must include 365 days of data. If your last physical was 60 days ago, it must include 60 days of data.

  • If you had a sleep test that came back negative for obstructive sleep apnea, we will need a copy of the completed test summary stating the sleep test was normal.

Diabetes:

  • Diabetic Patients: If glucose is present upon urine specimen testing, we will need a copy of a recent A1C that was collected within the past 3 months.

  • If you use insulin to treat your diabetes, Form MCSA-5870 must be filled out within 45 days prior to your DOT physical.

Heart Procedure

  • If you have had a heart procedure in the past, we will most likely need a cardiologist clearance along with cardiology test results. Please email us to request clearance form.
